What do we do?

Successful Transitions is for young people, aged 17 to 24 who are experiencing multiple barriers to education, vocational training or secure employment.

Services are flexible and include one-to-one support for all eligible young people.

The goal is to help young people to overcome the barriers and problems that make it difficult to stay in, or access, school, training, employment so they can fully participate in their community.

Talk to a Youth Worker today about your needs and see how they can help you with things like:
• Mentoring
• Case management
• Counselling
• Referrals to specialist support
• Community building
• Assistance and support with education or training
• Workshops
• Help during those hard times in life

Who can we help?

Eligible young people between the ages of 17 and 24
• not attending school, or chronically truant
• that are unemployed, but not ready or able to work
• who need help with their independence
• that need help with personal problems
